Position Summary

Metro Hands Home Care Services is seeking a compassionate, skilled, and dependable Registered Nurse (RN) to provide professional nursing assessments, supervisory visits, client education, and clinical oversight for home care clients. The RN will collaborate with physicians, caregivers, families, and interdisciplinary team members to ensure quality patient care while promoting safety, independence, and dignity in the home environment.

The ideal candidate must possess strong clinical assessment skills, excellent communication abilities, and the ability to work independently in a home care setting.

Essential Duties and Responsibilities

  • Conduct initial nursing assessments and ongoing reassessments of client care needs.
  • Develop, implement, and update individualized service plans in collaboration with physicians, clients, families, and the care team.
  • Perform skilled nursing visits and supervisory visits in accordance with state regulations and agency policies.
  • Monitor and document patient condition, response to treatment, medications, and any changes in physical or emotional status.
  • Provide patient and caregiver education regarding disease management, medication administration, infection prevention, wound care, safety precautions, and overall health maintenance.
  • Supervise caregivers and provide written instructions regarding client-specific care needs.
  • Ensure proper documentation of clinical notes, assessments, supervisory visits, incident reports, and progress updates in a timely manner.

  • Participate in on-call, weekend, and holiday rotation schedules as needed.
  • Maintain accurate timekeeping and adhere to agency attendance and dress code requirements.
  • Properly utilize and maintain agency equipment, supplies, and resources.
  • Comply with all federal, state, and agency policies related to infection control, safety, and patient care standards.

Qualifications

  • Active and unrestricted Registered Nurse (RN) license in the State of Florida.
  • Minimum of one (1) year nursing experience preferred; home care experience strongly preferred.
  • Current CPR certification required.
  • Valid driver's license, reliable transportation, and proof of automobile insurance required.

Position Summary

The LPN will work under the supervision of a Registered Nurse (RN) and collaborate with caregivers, physicians, families, and interdisciplinary team members to ensure clients receive safe, effective, and compassionate care.

Essential Duties and Responsibilities

  • Conduct supervisory visits and assist with ongoing client assessments as assigned.
  • Participate in orientation, in-service education programs, staff training, and teaching activities.
  • Implement and follow individualized service plans under the direction and supervision of the Registered Nurse.
